EDIT: Here are all the ports used by Black Ops 3 on PC and other platforms. Port forward all of the ones listed for your platform. TCP ports only need TCP forwarding and UDP only need UDP unless you see the same port in both fields then make sure to forward it both as TCP and UDP.
