I could still add/edit in the Field Maps App. I went back and re-created the form via popup in Field Maps, and that form was visible in the Field Maps App. I added more layers and changed basemap in the Beta Map Viewer and saved. There are two prefixes for the link that can be used: arcgis-survey123:// will open up in the Survey123 field app.
